In casual speech it means a lot of things, and people tend resent a person who thinks he is better than others, so all manner of individual interpretations are possible. Everybody makes up their own meaning, so everybody decides for themselves whether it is good or bad.

Everyone is of equal importance, but some people just have the idea that they are more important than others. This is completely untrue and makes both parties unhappy.
