How do you set up a live call in podcast with hosts in multiple locations?

So I want to start a live weekly podcast from Denver, with my co host in California and my other co host/screener in West Virginia. We need to be able

So I want to start a live weekly podcast from Denver, with my co host in California and my other co host/screener in West Virginia.

We need to be able to broadcast all three of our voices (screener when necessary) live over the internet, while still being able to take calls through the screener that will also be broadcasted over the stream with no lag.

What is the proper software and hardware we need to be able to do this? Cheaper is better, but any options you have will help!
