My boyfriend was in an accident. I'm his emergency contact, but no one called me. Why not?

We both have Kaiser insurance, he was taken to a Kaiser hospital, my name is clearly in his file, but no one called me. In fact, they didn't call ANYONE. He was all alone all night. What is the criteria for calling an emergency contact?

or:medical service staff on site often do not have the time to get a hold of any emergency contacts. They are busy, not only with the new patient, but other patients at the same time. So emergency contacts sometimes come later especially when the staff are trying to stabilize the patient. They could've been understaffed, so they were not able to call until things calmed down again and they might have forgot about it.
