Should I file for a Hostile work environment?

I have worked in the kitchen as a dishwasher at my local hospital for 3 years now and have had to endure the most infuriating co-worker ever! She is rude hostile and at times even threatening. One time I noticed we had a lot of dishes to clean up and decided to stay and help before I go on my lunch break. As I was helping her, she walked up to me and said "You better get out of here!". She came across as if I didn't she would have done something to harm me. I reported this incident to my supervisors (I don't know how they diciplined her) but afterwards nothing changed. An it is not just me who has problems with her too. At least 3 other of my co-workers have had issues with her and one of them is just downright afraid of her! I feel like the only thing left to do is file for a Hostile work environment or (worst case scenario) quit. What should I do?

or:Try reporting this co-worker to your supervisors next time she makes threats or makes someone feel unsafe or afraid etc. that behavior isn't acceptable at work. If your supervisors don't do anything to change her behavior then yes, file for an unsafe work environment. Don't tell your co-worker just file the form. Your supervisors should be taking this very seriously, I'm sorry if they're not. But definitely file an unsafe environment report if this behavior continues before you quit.
